The PHQ-9 is a self-administered diagnostic instrument for assessing the severity of depression in patients. It should only be used in consultation with a medical or mental-health professional.
The form utilises:
- Styling radio buttons as Buttons
- Custom JavaScript to calculate and display the final score once all questions have been answered.
- A User Driven Event to run the custom javascript after each question is updated.
According to the patient.info website: "The copyright for the PHQ-9 was formerly held with Pfizer, who provided the educational grant for Drs Spitzer, Williams and Kroenke who originally designed it. This is no longer the case and no permission is required to reproduce, translate, display or distribute the PHQ-9."
